University. At my small NAIA school, my school footed the bill for facial reconstructive surgery and custom masks.

Played NCAA D2 ball. If player is insured, the primary insurer of the player is used, and the school pays all copays/whatever else. The school also has a secondary policy to cover anything that may happen that isn't covered under your policy/if you are an athlete that doesn't already have insurance.
